  Teaching Skill Analysis Form  (updated for Fall 2009) This form is used to help students process their micro teaching after each individual lesson
  Summative Evaluation Form This form is used by UHS faculty and students to process and evaluate the entire clinical experience
  Summative Evaluation Suggestions Suggestions for the "other" category

Lesson Plan Analysis and Feedback Form This form will be used by UHS faculty and the clinical students to evaluate lesson plans before they are used to teach micro lessons.
Disposition Concerns Form This form is used by UHS faculty to identify and document problematic clinical student dispositions or attitudes.
 
Classroom Observation Sign-In This form will be used by UHS faculty in order to maintain a record of the clinical students who observe in their classrooms.
