Audit Request Form -- When a student audits a class, they are taking the course, doing the work, and fulfilling all course obligations as all other students, however they are doing so without receiving a grade or credit for the course. A student may audit a class or classes if facilities are available and class size permits. A student must have the approval of the instructor and their counselor. A grade of “AUD” will be listed on the student’s transcript. It does not affect grade point average or credits earned.
Grade Improvement Form -- The grade improvement policy encourages students to gain better knowledge of a particular subject area by repeating classes in which they experienced significant educational difficulties. It is designed to increase knowledge and mastery in a subject. It is only open to students who received a grade of “D” or “F” in the class they wish to repeat. Keep in mind, that in all cases, the second grade will replace the first grade, even if it is lower. An “F” will result in the loss of credit. Additionally, enrollment in a specific class will be dependent upon the availability of room in that class. Students taking the course for the first time have priority.
Pass/Fail Request Form -- The Pass/Fail option is available to sophomores, juniors, and seniors. It may be used for a maximum of three credits toward graduation requirements and a maximum of one credit per school year. The Pass/Fail option must be selected within the first 10 days of the requested semester, however students may elect to return to a letter grade at any time during the first four weeks of a semester. In choosing this option, either a grade of “P” or a grade of “F” will be recorded on the student’s transcript. A passing grade will not be included in the computation of GPA, however a failing grade will be included. Please note that the recording of “P” or “F” is only completed at the end of the semester, therefore letter grades earned during the first quarter of a semester will be recorded on the student’s report card.
Heartland Community College -- Students may enroll for classes at Heartland Community College with the recommendation of a U-High teacher and the permission of the U-High Counselor. There are four pages to this form. The first page must be returned to the Counseling Center and the remaining three pages go to Heartland. One important note is that the fourth page is only required for Spring semester classes.
ISU Dual Enrollment Application-- Students may apply to take courses at Illinois State University for college credit. Students will be expected to pay ISU tuition for the class and it will appear on both a U-High transcript and an ISU transcript. Once accepted, a meeting will be set up with the University College Advisors to complete the registration process. Once registration is completed, student will need to complete the ISU Dual Enrollment Registrar Form.
ISU Dual Enrollment Registrar Form -- This form is completed once a student has registered for an ISU Dual Enrollment course.
ISU High School Credit Course -- Students may request enrollment into Illinois State University courses for high school credit only. A recommendation from a U-High teacher and permission from the U-High counselor are required. Once obtained, student will then make arrangements with the ISU instructor and gain their permission to take their class. Since there is no college credit involved, there is no tuition charged.
IVHS Request Form -- Students wishing to take an on-line IVHS course, must first discuss the options with the counselor and then complete this form. Please note that the course must be paid for in advance and that the student will have to register on-line at www.ivhs.org in addition to completing this form.
Independent Study Contract -- Independent Study arrangements are available for sophomore, junior, and senior students. They may be for credit or non-credit and may be taken pass/fail. If credit is to be earned, contract and preparation time should roughly correspond with a Carnegie unit of credit, which is one hour per day, five days per week for ˝ credit per semester. Independent Study is offered only in courses not offered in the curriculum. A student may not carry more than one Independent Study per semester and may not apply more than three credits toward graduation requirements.
Intercity Request Form -- This form is for use by any student (U-high or non-U-high) who is taking a class at a school other than their own. Other school students taking a class at U-High must complete this form as do U-High students taking a class at a local school other than U-High. A copy should be given to counselors at both schools.
PE Waiver Request Form -- Juniors or Seniors who meet the eligibility criteria for PE Waivers need to complete this form for the semester in which their waiver applies. One form is required for each semester. Students who do not have this form completed must be scheduling in a Physical Education class.
Teacher/Office Assistant Request Form -- A student may elect to spend one of his/her six required daily classes assisting a teacher or working in a school office. This is a mutual agreement on contract between a teacher/supervisor and the student. The student agrees to be on time daily to perform tasks given by their supervisor. No credit will be given.
UM CDIS Request Form -- Courses offered through the University of Missouri Center for Distance and Independent Study are available with Counselor approval. This form, and registration at www.cdis.missouri.edu are required.
Information for Recommendations -- All seniors should fill out this form for the counselor and/or teacher(s) who will write letters of recommendation.
Consumer Education Proficiency Test Form-- All High School Students. Fill out and return to the Counseling Center no later than Monday, September 24th, 2007. Test will take place on Wednesday, September 26, 2007.
